8.2 Domestic Partner Coverage

At Play-Well TEKnologies, we recognize that some of our employees are members of families that do not meet the traditional definition of the word. That is, a husband, wife, and perhaps, children. For those employees who are not married but who are in a committed relationship with another adult, we provide domestic partnership coverage. Employees who are eligible can choose to add coverage for their domestic partner. To be eligible for benefits, the employee and the employee’s partner must meet *all* of the following criteria:

  • They must have lived together in an exclusive committed relationship for at least 12 months.
  • They must be at least 18 years of age.
  • They must live together in the same residence.
  • They cannot be legally married to, or in a registered domestic partnership with anyone else.
  • They must not be related by blood; more closely than would be allowed under the marriage laws of their state of residence.
  • They must complete and sign a Domestic Partnership Affidavit.
