16.3 Please Notify Us if Your Information Changes
Because we use the information in your personnel file to take actions on your behalf, it is important that the information in that file be accurate. Please notify the business office if you change any of the following personal information:
- your name
- mailing address
- email address
- phone number
- dependents
- marital status
- restrictions on your driver's license
- the number of dependents you are designating for income tax withholding
- the name and phone number of the individual whom we should notify in case of an emergency
