8.4 Flexible Benefit and Commuter Benefit Plans

We offer employee-funded Medical Expense, Dependent Care Expense, and Adoption Assistance Expense flexible benefit plans for employees with at least one year of employment who have worked more than 1,000 hours since January 1st of a calendar year and are at least 21 years of age. Eligible employees can choose to enroll on the first date of the second month after reaching eligibility, or during the 'Open Enrollment' period which begins October 1st.

Flexible benefit plans let you pay for qualified expenses through payroll reduction, using pre-tax dollars. Contributions to the dependent care expense plan are limited to $2,500/ year; contributions to the medical expense reimbursement plan are limited to $5,500/ year; contributions to the adoption assistance expense plan are limited to $12,970/ year.

San Francisco Medical Reimbursement Plan
Employees who have worked for Play-Well for at least 90 calendar days and work an average of at least eight hours/week within San Francisco city limits in any given quarter are eligible for a Medical Reimbursement Account (MRA) funded by Play-Well, through the City of San Francisco, under the Health Care Security Ordinance (HSCO.) An MRA is an account established to reimburse you for your out-of-pocket medical, dental and vision expenses. These expenses are not limited to service providers in San Francisco.

Expenses that qualify for reimbursement include any out-of-pocket medical, dental or vision expenses that are not being reimbursed from another source (insurance carrier, flexible spending or health saving account, spouse’s employer, etc.) for you and your eligible dependents. Eligible dependents include your spouse and any dependents that you claim for tax purposes.

We deposit $2.53 per hour worked (within San Francisco city limits) into the MRA account at the end of each quarter, minus the cost of any employer contribution we made toward the employee’s health premiums. San Francisco sends out quarterly statements to eligible employees showing account balances and how to claim funds. MRA funds remain available for 24 months from the date of deposit.

Eligible employees who end employment with Play-Well can be reimbursed for claims incurred up through the quarter in which termination occurred; claims must be submitted within 90 days after the quarter in which termination occurred. Unused funds are forfeited.

San Francisco Commuter Benefit Flex Plan
Employees who work an average of at least ten hours/week within San Francisco city limits during a calendar month are eligible to participate in the Commuter Benefit Flex Plan on the first day of the second month following. Enrolled employees can choose to have up to $145/month deducted from their pay in pre-tax dollars and submit claims for reimbursement of paid public transportation for commuting to work for Play-Well within San Francisco.
